Page 1 - Personal Information
This page gathers info on the client if they take the survey on your website.
But if you send the invitation to take the survey via email, the form can be automatically pre-loaded with any data. E.g. stylist name, date of visit, and service codes. This data can even remain hidden from the client but be part of the results.

Answers (e.g. Gender) can be used to change the questions asked further into the survey.
Page 2 - Your Visit
The Overall rating on this page has logic attached i.e. if the answer is Good/Very Good - do this. But, if the answer is Poor /Very Poor - then do this.
The ability to change the survey based on the answers is very powerful for getting the answers you need about your Salon and its clients.
You could even ask different questions for client that had a Cut or a Colour
Page 3 - Good News
This page will only appear for Good/Very Good Overall ratings
You can ask for positive feedback about their visit and this "testimonial" can be automatically posted to a page of your website.
You could also post this comment to your Salon Facebook Page - or even the Facebook Wall of the person completing the survey!

The final questions lets them "recommend a friend". This triggers an immediate email to their friend with a Discount Voucher.
Page 3 - We are sorry
This page will only appear for Poor/Very Poor Overall ratings
The client can provide further information as to why they are not happy - and this can trigger an immediate email to your salon for follow-up
The client gets an immediate email telling them you are dealing with the issue they raised. As a "survey gift" may not be appropriate, we now has a jump to the Thank You page at the end of the survey

Page 4 - Reward for the Guys/Girls
This page will only appear for Good/Very Good Overall ratings and varies for each Gender.
This means we can offer different "thank you gifts" to guys and girls. Or it could be based on any other answer - e.g. age, regular/new client, type of service they had...
The "gift" is automatically sent to them as an email Voucher so that you can track usage in-salon

This page:
- complete the survey
- saves all the data
- sends all the emails based on triggers/answers
These can be formatted to show only the information you want in the way you need it.
